BucketList +1 is an interactive awareness campaign that provides resources to small and large communities to foster comfortable conversations around death and dying.

Remember — death is inevitable. By communicating our wishes for end-of-life healthcare, we relieve our loved ones of the stress and worry of making decisions on our behalf.

Decisions Day

The campaign culminates in Decisions Day, a positive and affirming event where individuals make their personal wishes known by

signing and notarizing a completed advance directive (Durable Power of Attorney for Healthcare).

The BucketList +1 campaign offers four engaging steps to
advance care planning:

Each step features optional activities and events that can be tailored to the interests and information needs of the group.

We inspire you to TALK with friends and family about your wishes and select a healthcare agent.

We help you LEARN advance healthcare planning terminology.

We provide you with the tools to PLAN for end-of-life decisions.

We help make your wishes official and NOTARIZE an advance healthcare directive.

Plan of Action

We help you select activities within the 4-step campaign action plan and create a schedule and budget.

Rest assured, you can implement the plan with a few volunteers and minimal funds. Our key tool is the orange #MyDeathMyPlan packet, an advance care planning toolkit.

To help jump-start your campaign, we provide our partners with a refreshment credit to support a facilitated conversation about advance directives.

Decisions Day

Your campaign builds toward Decisions Day,  a date of your choice where community members can sign and notarize an advance directive.

This is a day for your community to show you “Donut Fear the Reaper” by making your medical wishes known.

While advance care planning can happen at any time, we invite partners to schedule the Decisions Day event on or near April 16 – National Healthcare Decisions Day!
